Funeral Directors Advisory:

At Legacy we highly recommend and encourage the Next-of-Kin and or Authorizing representative/s to gather and have accessible, as much as possible, the following vital information and or items for the deceased prior to making arrangements.

  • Social Security Number
  • Military Discharge - Form DD-214
  • Date of Birth - City & State
  • Parents Full Legal Names - to include mothers maiden name
  • Obituary Information
  • Photograph
  • Clothing - to include undergarments



Obituary Information:

Legacy has put an Example & Obituary Worksheet together to assist you in writing your obituary notice. We have also put together an Obituary Information Page from the ADN you may find helpful. We encourage you to visit the ADN's Obituary Link for current and updated information regarding ADN's submittal and payment processes.

Please note we will be happy to honor your loved one/s by posting their obituary notice on our website which you may share with you family and friends by providing them the link. We will send you their obituary notice link when completed. Please allow 3 to 5 business days from receipt of submission for Legacy personal to finalize and upload.
